Policies / Enrollment Agreement
Studio & Office Hours
The Front Desk will be open Monday through Thursday 2:30-9:30pm and Friday 2:30-6pm. Parents
please note: the studio doors will be locked at 9:30pm on Monday through Thursday for the safety of
our students and staff regardless of class end time. The Front Desk will NOT be open on Saturdays.
Please take care of all business issues during our regular hours Monday through Friday.
Season 6 Studio Holidays (Studio Closed)
September 5, 2011 (Labor Day)
November 22-26, 2011
(Thanksgiving – We will be open Monday, the 21 for all classes)
December 19, 2011 - January 1, 2012 (Christmas)
March 12-17, 2012 (Spring Break)
Last day of classes is May 26, 2012. Please note that tuition is due in full for each month regardless of whether it is a holiday month.
Tuition & Payment Policy
1. A $40 registration fee is charged for the first family member and $10 for each additional family
member per season and must accompany the registration form.
2. All tuition is due on the first week of each month. We do not mail out monthly statements but reminders are posted at the studio and email reminders sent. A $15 late fee will be added automatically to all accounts paid after the 10th day of the month. Full tuition is due each month regardless of the number of classes or lessons. Tuition is not pro-rated for absences or holidays
with the exception of August, which is half tuition. When making payments, PLEASE include
student’s name and how the money should be applied to your account on the memo section of your
check (example: “Susie Jones, August tuition” or “Susie Jones, costume fee”). Payments may be
mailed or taken care of at the Front Desk. Some of our families make arrangements with their bank
for automatic bill pay. To avoid a late fee or as a convenience, payments may be made by
phone with a credit card. Cash, Checks, Visa and MasterCard are accepted for all payments.
3. Additional fees such as recital fees, costumes, art supplies, etc. are due at various times throughout
the year.
4. A $30 service charge will be added to the account for returned checks.
5. NO REFUNDS.
Withdrawal Policy
Enrollment in classes or lessons at Lifesong Studio & LifeStage Theatre is through the end of May.
The Front Desk must be notified of withdrawal from any class or lesson prior to the beginning of the
month in order to stop tuition for the next month. All withdrawals are written in our “Add/Drop” book at
the time of notification. Failure to properly notify the Front Desk will result in tuition continuing to
be charged to your account. By enrolling in a class or private lesson, you have reserved a spot that
could be released for someone else. Thank you for your cooperation and understanding of this policy.
Studio Rules & Guidelines
Please help us maintain the quality and integrity of our studio atmosphere by observing the following:
1. Be respectful and courteous to everyone including staff and other students.
2. Keep the noise level to a minimum in the student lounge and hallways while classes or lessons are in
progress.
3. Dress appropriately while in the studio.
4. Keep food and drink in the student lounge area only. Water bottles only in the studios.
5. Clean up after yourself both in the studios as well as the student lounge.
6. Silence cell phones and pagers during classes or lessons.
7. Supervise young children.
By enrolling in classes at Lifesong Studio and LifeStage Theatre, I acknowledge that I have read and agree to
abide by ALL studio policies. In consideration of Lifesong’s commitment to maintain a safe facility, I release and hold harmless Lifesong Studio, its employees, administrators and contractors from any and all liability, whether for personal injury, property damage, or otherwise by my family’s participation in activities in the Lifesong Studio
facility. I also release permission for photographs to be used in Lifesong promotional materials including, but not limited to: website, brochures, posters, advertisements, and programs. I have read the above information and agree to abide by the policies as stated.
