POLICIES

Lifesong Studio & LifeStage Theatre Policy Agreement:
Upon registration, you will be asked to agree to policies through your parent portal.

Registration Fee Policy
A non-refundable registration fee is required for all students upon enrollment in each season (mid-August through May). Regular season registration fee is $40 for the first family member and $10 for each additional family member.

Tuition Policy
Tuition is due in full on the 1st day of each month regardless of the number of classes/lessons. Tuition is pro-rated ONLY if a student begins after the first week of the month leaving less than 4 lessons in that month. NO CREDIT for absences, holidays or studio closing. NO REFUNDS. NO EXCEPTIONS.
We require a credit or debit card (Visa, MasterCard or Discover) be kept on file with authorization for tuition payments between the 1st and 5th day of each month. Any failed transactions will be charged an additional $25 and enrollment will be suspended until payment has cleared. This authorization will be in effect until the end of the current season or until Lifesong receives written notice to withdraw.

Additional Fees Policy
Miscellaneous fees such as recital & costume fees, art supplies, etc. are due at various times throughout the season and will be charged to your card on file when due. Payments for these fees may also be made by check, however the check must be received prior to the due date in order to keep your card from being charged. A $25 service charge will be added to your account for returned checks. No credits. No refunds. No exceptions.

Withdrawal Policy
Upon enrollment in the regular season, you will remain enrolled through the end of the season (May). IF YOU WANT TO DROP, YOU MUST GIVE WRITTEN NOTICE 30 DAYS PRIOR TO THE BEGINNING OF THE MONTH in order to stop the tuition billing process and keep your card from being charged. Failure to give written notice will result in tuition being charged to your account and automatic payment will be processed without refund. Click HERE to withdraw enrollment. Bad Weather Policy
We typically follow the Grapevine-Colleyville ISD closing for bad weather. We will email confirmation of bad weather closing to our families so check your email if in doubt. We will also post closings on our website at www.LifesongStudio.com as well as various Lifesong social media.

Attendance/Make Up Policy
There are NO REFUNDS OR CREDITS for personal absences. Due to limited options and limited space, we do not typically offer makeup classes. Please make every effort to be in class for your own benefit as well as the benefit of the class.

MUSIC ATTENDANCE/MAKE-UP POLICY
Tuition for private lessons is a reservation of time and guarantee of a standing weekly appointment with your teacher. There are two kinds of absences – EXCUSED and UNEXCUSED.

  1. In order for an absence to be EXCUSED and eligible for private make-up, you must contact your instructor by text or email directly by 12pm on the day of the scheduled lesson, although we appreciate as much notice as possible for any planned absences. Calling or emailing the Front Desk/Studio does not fulfill this requirement. Each student will be allowed 6 makeups per calendar year for excused absences in music lessons.
  2. An absence will be considered UNEXCUSED if notification is made after 12pm on the day of the scheduled lesson or if there is a “no call” or “no show.” These will NOT be considered eligible for make-up.
  3. Lessons will be automatically terminated if a student misses 2 consecutive lessons with no communication to the teacher.
  4. Missed lessons due to the teacher’s absence will be rescheduled for a private makeup.
  5. NO REFUNDS. NO CREDITS. NO EXCEPTIONS.

Observation Policy
Parents will be invited to observe classes at various times throughout the season. An email will be sent prior to “observation days” and signs will be posted at the studio.

Dance & Theatre Private Instruction Policy
Our staff is available for private instruction in dance and theatre as needed based on studio and teacher availability. Privates are handled between the instructor and the student. Contact the front desk for information regarding private instruction.

Class Cancellation Policy
Lifesong Studio reserves the right to cancel or combine classes if the class enrollment drops below 5 students.

Dress Code Policy
Dancers must wear a cover-up over dancewear at all times except during class or rehearsal. Clothing for all students should be modest and neat in appearance and void of any offensive words or pictures.

Lifesong Studio & LifeStage Theatre Behavior and Discipline Policy 
We are committed to having a safe, nurturing and fun place where children can be creative and use their imaginations without limit. Students are expected to participate in all classes, rehearsals, lessons or workshops for which they have enrolled with a positive attitude, showing respect to staff and other students. In the event that negative or disruptive behavior becomes a problem, we will follow our “Strike Policy” as listed below:

  • Strike One: Verbal warning from instructor/director and possible chat with instructor after class. Strike One will be documented by the instructor.
  • Strike Two: Sit down chat with instructor/director. This meeting will be documented with a behavior plan and parents will be notified of the situation via email and/or phone call.
  • Strike Three: Parents will be called and the child will be removed from the class or show (in the case of a theatre production) and will not be allowed to return for the remainder of that class or show. No refunds in the case of Strike Three. 

Business Hours
Our staff will be at the Front Desk and available to assist you in the regular season during the hours of 2-7pm Monday through Friday and Wednesday mornings from 10am-12pm. The Front Desk will NOT be open on Saturdays and Sundays. Summer hours vary and will be posted on the website.

Holidays
Lifesong will be CLOSED in observance of the following holidays:
Labor Day
Thanksgiving week (Monday through Saturday)
Christmas Break (2 weeks per GCISD schedule)
Spring Break (per GCISD schedule)

Right of Refusal Policy
Lifesong Studio reserves the right to end enrollment regardless of payment and/or ask anyone to leave who is not behaving in the best interest of the studio, staff, other students or families.

WAIVER AND RELEASE OF LIABILITY

In consideration of the risks of injury while participating in dance, music, art and theatre instruction (collectively the “Activity”) at Lifesong Studio (the “Studio”), and as consideration for the right to participate in the activity, I hereby agree as follows for myself, my heirs, my executors, administrators, next of kin, successors, assigns and personal representatives.

Waiver and Release: I hereby waive, release and discharge any claims, liabilities, demands, causes of action or rights of action of any kind whatsoever arising out of my participation in the Activity, and do hereby release and forever discharge Lifesong Studio, it’s owners, instructors, employees and agents (collectively the “Lifesong Studio parties”) for any physical or psychological injury including but not limited to illness, paralysis, death, damages, economic or emotional loss, that I may suffer as a direct or indirect result of my participation in the Activity, including traveling to and from an event related to the Studio or Activity.  

Express Assumption of the Risk; I am aware of the risks associated with participating in this Activity, including travel to and from said Activity, which may include, but are not limited to, physical or psychological injury, pain, suffering, illness, disfigurement, temporary or permanent disability (including paralysis), economic or emotional loss, and death. I understand that these injuries or outcomes may arise from my own or others' negligence, conditions related to travel, or the condition of the Studio and/or Activity location(s). Nonetheless, I assume all related risks, both known or unknown to me, relating to the participation in this Activity. I am voluntarily participating in the aforementioned Activity and I am participating in the Activity entirely at my own risk.

Indemnification; I agree both not to bring any released claims as described herein and also to indemnify and hold harmless the Lifesong Studio parties against any and all claims, suits or actions or any kind whatsoever for liability, damages, compensation or otherwise brought by me or anyone on my behalf, including attorney’s fees and any related costs, if litigation arises pursuant to any claims made by me or anyone else acting on my behalf. If the Lifesong Studio parties (or any of them) incurs any of these types of expenses, I agree to reimburse the Lifesong Studio parties for those expenses. 

I acknowledge that Lifesong Studio and its owners, directors, officers, volunteers, representatives and agents are not responsible for errors, omissions, acts or failures to act of any party or entity conducting a specific event or Activity on behalf of Lifesong Studio.

By signing below, I understand, acknowledge and agree to the following: 

I acknowledge that I have carefully read this waiver and release of liability and fully understand that it is a full and general release of liability of both known and unknown claims.

In the event that I should require medical care or treatment, I agree to be financially responsible for any costs incurred for such treatment, including emergent treatment, if any, reasonably required while participating in the Activity. I am aware and understand that I should carry my own health insurance.

This Agreement is entered into voluntarily, without duress or coercion, and is to be interpreted as an agreement between two parties of equal bargaining strength. I confirm that this Agreement is clear and unambiguous as to it’s terms, and that I have had an opportunity to ask questions and/or seek clarification regarding any and all provisions. No other evidence will be used or admitted to alter or explain the terms of this Agreement, but that it will be interpreted based on the language in accordance with the purposes for which it is entered into.

This Agreement will be governed by Texas law and any claims under or related to it must be brought exclusively in state or federal courts in Tarrant County, Texas.  In the event that any provision contained within this WAIVER AND RELEASE OF LIABILITY shall be deemed to be severable or invalid, or if any term, condition, phrase of portion of this Agreement shall be determined to be unlawful or otherwise unenforceable, the remainder of this Agreement shall remain in full force and effect, so long as the clause severed does not affect the intent of the parties. If a court should find that any provision of this Agreement to be invalid or unenforceable, but that by limited said provision it would become valid and unenforceable, then said provision shall be reformed and deemed to be written, construed and enforced as so limited.

In the event that the participant(s) is under the age of consent (18 years of age), then this release must be signed by a parent or guardian, as follows; I hereby certify that I am the parent or guardian of the participating student(s) on this account and do hereby give my consent without reservation to the foregoing on behalf of this individual. If I am signing this waiver for my child, I certify that I am the parent or legal guardian and have the right to waive these rights.

I HEREBY AFFIRM THAT I AM EIGHTEEN (18) YEARS OF AGE OR OLDER, HAVE READ THIS DOCUMENT, UNDERSTAND IT’S CONTENTS AND I HAVE FULL LEGAL CAPACITY TO SIGN THIS DOCUMENT.
